Error while trying to retrieve any unity form template

Hello there,

I got this error every time I tried to retrieve unity form template:

An error occurred within the Unity API: A field with the same ID already exists on the form.  Fields must have form-wide unique IDs.

can anyone help?

Is it possible that you do have a duplicate keyword? Does the Template have a keyword as a MIKG?

No Daniiel, all keywords are stand alone
I tried to retrieve other templates but I got the same error

I have learned that this is possibly caused by database corruption and the Unity Form has been compromised and needs to be rolled back to a previous working version before you will be able to access it again. It is recommended that you contact your first line of support to review your database to assess if this is the case.
