WorkView Data Provider

I'm trying to configure a WorkView data provider in v18 SP1. Previously, we used the Custom SQL since this option wasn't available. When first create it, all i get is an empty screen with no options to select:

Is there an additional setting that needs to be set on the WorkView class/filter to get this screen to populate? The v18 MRG for Reporting Dashboards and WorkView both say to use the Custom SQL Provider and the new WorkView one isn't referenced anywhere that I can see.
