ESRI Intergration with Unity Forms

Has anyone used a unity form to "map to" with ESRI integration?


Instead of using a document already with in OnBase to "map to" we are wanting to create a new project notification form that will hold project information that can be submitted to hit a workflow & notification process & be "mapped" to one of our interactive maps on the GIS side. We are upgrading our testing system to 18 to take advantage of the latest updates for the ESRI integration but have not come across any discussions on forms being used for this instead of documents already indexed in OnBase.

I have used shared Unity Form as a popup link in GIS side.

Thanks Shankar! We are trying to set this up but are a bit confused as to how to configure to have the "unity form"  to be available to fill out as the user plots the pin point on the map of the location. We are a forest preserve district and don't really have formal addresses to utilize the "geolocation" functionality as the locations don't have mailing addresses and in certain instances we could be selecting a polygon that the fillable "unity form" would associate to for projects like clearing or burning. Would you have a snap shot how we can do this? Is it configured through the OnBase widget tools? Everything is installed for the integration to work properly and running v18 on our test system but having trouble figuring out all the configurations. Based on the ESRI integration for v18 this should be possible.
