A sporadic error is generated during an indexing step in a workflow...causes the document to get stuck in the que. There is an Ad-hoc task option that allows the users to manually index the document (which seems to work fine) but the problem is when the document runs thru the workflow, it chokes at the indexing step in the workflow. Does anyone know what could be causing this error? (screenshot is attached)

The error indicates a problem in the low level communications with the OnBase database. A common cause of such problems is incompatible ODBC Drivers
So the first thing I would do is to verify that you are using the correct ODBC Drivers for your system.
If the drivers are correct, or you're not sure which are the correct ODBC Drivers, I recommend contacting your first line of support for further assistance

Are you by chance using a CORE based action but your client is in classic mode for workflow on the workstation.
Try to identify if this happens when you do the step using unity OR if in the thick client with core
OR vice versa
