Charts requiring review function under the Medical tab in the Client

I'm looking for documentation of what qualifies a chart to fall under the "charts requiring review" option under Medical in the thick client. Also, when one is revised - what are the transactions that are triggered and where do they update?

Hi Melissa,

The reasons a chart might require review are documented in the Medical Records Unity Client and Signature Deficiencies for Epic Module Reference Guides under the "Reasons Charts Require Review" section.

Here are the complete reasons you will find.  Based on your solution, only a sub-set of these might apply.

  • Missing MPI
  • Missing MRN
  • Missing Chart ID
  • Incorrect format for Admit Date
  • Incorrect format for Discharge Date
  • Incorrect format for Date of Birth
  • Unknown or Missing Medical Facility
  • Unknown or Missing Admit Type
  • Attending physician is invalid, and the facility requires a valid attending

From a transaction standpoint, the chart record in the database will be updated to update the flags that indicate that the review is required and the reason for the review.  The values that were updated during the update process will of course occur in transactions as well.
