Document correction

Hi , 

I have question for document corrections. I had a request to remove a lab result which had been scanned. After I removed it, I just realized that I should enter the reason why it had been removed such as "mislabelled" or "errneous". Can you please advise what is the proper steps to accomplish this? If i can go back and enter the reason. Please note, this is an urgent request

You should be able to retrieve the original document and add the reason to the note that gets added to the corrected document.
