Does an organization have to be on Foundation to utilize the Simplified Licensing Model? Or can they be on 18, for example?

Sorry for the delayed response. Yes, in order to prevent unforeseen license consumption challenges in certain scenarios, as well as provide more streamlined administration, it is recommended customers be on Foundation PRIOR to flipping to Simplified Licensing.
