We are using NTLM. As I mentioned the problem is not consistent either. But it is not the highest priority issue i have so I will try to replicate and figure it out, if possible.
I am very new to IM/KM/ECM so I think the answer is yes, I have put a rule in place that every document is versionable. There are other rules in place that categorise documents etc, but I have nothing advanced in place. Would this cause the problem?

I may have not explained myself properly here, i do apologise. What you say is correct - users are indeed editing documents locally and then publishing/uploading the newer versions.
What I want to do is hide the "upload new version" button UNLESS the user has chosen to edit offline - thereby locking them into the correct process - at present you can upload a new version at a whim - you do not have to check the document out. So i don't want it turned off - i want it hidden unless certain conditions are met i.e. user must have checked out the doc. The only place this button should be visible is when viewing a document in the "I'm Editing" folder.
I don't understand why you can just upload a new version - without having check it out - this can lead to confusion and work being overwritten. I want our users to have to check it out to be able to upload a new version - does this make sense?
1. Some users are being re-prompted to enter their credentials when they click on upload. This is only seen in Firefox and has only started happening recently i.e. it did not always do so, it also does not happen for all users.
Are you using out of the box authentication or NTLM?

3. .docx files, etc (so anything from MS Office 2007) does not display properly in the document preview - is there a fix for this or must i ask users to only save their docs in the earlier format
Alfresco uses open office for the conversion from .docx to PDF for the Flash Preview. How do the files look when you open them in Open Office directly?

4. why does the "upload new version" button display by default, this seems counter intuitive to document management, surely the button should only be visible IF you have selected to "edit offline", hence the button will only be visible to the editor, within the "I'm Editing folder"?
5. if the answer to point 4 above is that the button should be there (I don't quite understand why though, the only instance you want to upload new version should be if you have edited doc) is there anyway I can hide the button from view, only visible to the editor?
Users might be editing documents locally and might be using Share for publishing / sharing documents. Yes, it can be turned off. Having it on or off really depends on your document management process.
