Error on new tag

Hello
i'm trying to add a new tag to a document and i'm getting this error:

has anyone had this problem before ?
i had an installation of version 4.0 on windows and i didn't had any problem. but now on new installation on Ubuntu server 64x i'm having this problem.

Problem solved only after a re install.

But why this kind of issues are there? I also faced this a few times. A number of times I need to reinstall…
