'Raise an issue' link

i would like to create a link next to the help button called "Raise an issue" just like we have in the web explorer. which file do i use for this?

Thanks mike, the link worked. i would like to call a local file using that link through a web browser( when you click on the link it opens a file located under my drive C: in my computer) i have tried file:///Cfilename) but it does not work. have a workaround for this?
