How to change title and .ico in all activiti pages?

Hi Experts,
Please tell me how can i change the default title "Activiti Explorer" and .ico file to my custom title and .ico.

I have searched all over the application but couldnot able to find where this title is assigned to the pages.

Please Help………….!

This is not related to Activiti at all, simply search for changing the .ico in any Java webapp.
