UI not rendered as per formKey

Hi Guys,

What am I missing here? FormKey has been defined and also the forms directory contains the necessary file. Yet in Activiti Explorer (5.11) the task's UI is not rendered as per the form.
Attached you can find it as a Maven/Eclipse project. I am running this on Windows 7, but it does not work neither with \ nor with / as path separator.

PS: why is so that in an Activiti project in Eclipse the maven "package" goal produces a jar instead of bar? What should I change to produce bar? Currently I only rename jar to bar.

PS2: I got a "Could not upload attachment to ./files/5448_ff0798672084451c7a5c0ad7b8364547." for upload  Smiley Sad .

PS3: project is pretty simple - after deployment:
