System Error : found more than one user for xyz

While trying to change password for user get the following error msg-

A system error happened during the operation: Found more than one user for xyz (case insensitive)

As admin, I do not see more than 1 user for xyz…. why does this happen and how does one fix this error.

Using 2.1 OS Edition.

Are you using any particular authentication mechanism, such as LDAP or NTLM?
