Populate action form with data from custom datalist

I have a custom datalist with an association field to a document in repository.

I would like to build an action from document options which retrieve the datalist and then fill a form. The form, if saved, will modify the datalist.

Building an action and add link to document with all relevant evaluators is not a problem. Instead I don't know how to pre populate the form.
