How to create Form

Hi,

fisrt tks all for this great forum and this ECM.

so i m novice, and i have a Alfresco 3.2 community installed.

i want to create web form. when i said web form it's web form with enter your name, description etc… like this http://wiki.alfresco.com/wiki/Forms_Examples

i have read this wiki but it don't understand how i can create this type of form. How i can insert into alfresco this form…

if anybody can help me about this..
