How to create form using 'Alfresco form model management pl'

hi all,
      this is venkat and newbie to alfresco and forms.how to create and manage forms using "alfresco form model management".If anybody is there using this plugin please help me create forms in share.please give me step by step instructions to create forms and from how and where to call the created forms in share.Using webinar i am not able to create forms completely…what changes we make in share to using this plugin…

         yes i have done to create form using form model management, so i will create a form with two fields like username and password, and i want that form is execute in within one dashlet.how is it.now my doubt is how we maintain state of that form and functionality to store or retrive data from database.
