is possible to create a custom template preview, and if so how?

Go look at the many examples available in this folder in your repository:
Company Home / Data Dictionary / Presentation Templates

Then create a copy of one and modify it as you see fit.

Then select it from the list when you apply a template to the folder or document in Alfresco Explorer.
