Form not displayed for activiti:formKey

Hi ,

I want to display a form in start event.
In \activiti-modeler-examples\MySampleflow folder i created my own form named "Sample.form".
And in mysampleflow.bpmn20.xml i gave the activiti:formKey=""Sample.form"

But after i deploy my Sample.Bpmn20.xml file when i click Showstartform form it says "failure".
What am i missing?

How to display my form ?

Is the form deployed alongside your bpmn20.xml file? And make sure the file and path-name (relative to deployment archiver root) are correct.
