Activiti Explorer shows 404

I am using activiti 5.12.1.
I have already deploy a bar file .
Now if i going click on "convert to editable model" for a process ,
it shows "HTTP Status 404 - Not Found".

also if i try to create a New Model in Explorer and select "Activiti Modeler" , this one also give  "HTTP Status 404 - Not Found".
The "activiti-modelor" jar already added in "activiti-explorer/WEB-INF/lib/"
