activiti-app is not being launched by using following url http://localhost:8080/activiti-app

I have done all the pre-requisite work.But activity explorer is being launched but activiti-app is not being launched.what can be the reason behind it?
and is there any support of forms integration with tasks in process in activiti explorer???
