How to create a Custom hierarchical workflow?

I want to implement  a custom workflow which work in hierarchical manner without using database. Like if it is approved at first level then it goes for approval at second level, finally goes to third level to get final approval. Workflow will be visible only to those are approving it.

Any suggestion?
