How Deployed Form and Linked that form with Process.

I am working on spring boot application with Activiti Ui 6. But their is no option to Form Deployment but when I have create Form at http://localhost:8080/activiti-app/editor/#/ and linked that Form based on Key in process then On Ui then When task is load to Assigned user it display exception.

Bump.  I also have this issue with Activiti UI 6.  I can create forms but cannot seem to deploy them.  When I pull up my user tasks, it throws an error stating the form I'm looking for is not deployed.

I seem to have this problem as well, wondering if there will be a patch to handle this functionality.
