Adds Document Library Toolbar 'Create Content' - Menu items' for direct document creation via MS Office

This uses the same ActiveX and browser plugins as the MS Office the standard edit-online feature. ActiveX - OpenDocuments?

There are many office versions and quircks around how they interact with the sharepoint protocol, so expects some differences in behaviour. MS Office 2011 for Mac is known to have an issue with not showing the Alfresco folder when saving, so there is a work-around, using copy-in of file and launching of edit-online instead.
