Why, when I edit a document, am I identified as "System" while other people are identified by name?

Can you please provide some extra details as to how you are editing? where you are editing ? 

Are you uploading new version of content or editing properties? What is your role in the site (if edits are within site)? Outside of site every user is default consumer unless externally provieded an specific role.
