Forms and Documents in Activiti 7

I have following doubts which I could find in the documentation.

1. Where is the document uploaded/created in the process in Activiti 7 is stored?

2. How to create and deploy the forms? And attach to user task or start task in modeler app?

3. How to pass forms variable to the connectors?
