Connect Forms to BPMN Model in Activiti Modeler

i’m using the Activiti Modeler and want to ask if there is a feature to connect forms to a task in the BPMN model? I saw this feature in Alfresco Process Services.

Activiti 6 comes with a form designer in the war version. eclipse plugin only provides bpmn modelling.
