I apologise before hand that I am repeating this post as the past thread has gone on too long and I think I need more eyes to look at it and please profer a solution.
Thanks to @abhinavmishra14 who has tried to help me capture the logs.
The "insert" featue in the pdf toolkit does not work and gives an "insert pdf operation" error, with the help of @abhinavmishra14 I have captured the logs as best as I can using the debug code. Please find the logs below:

What could be the problem here and how can I fix it so that the "insert pdf" feature can execute successfully.
