How do we connect a form with a user task in a process in activiti app?

So i have tried working with Activiti App now. 

I design a form and give it a form key.

When i make a process with usertask and inside the user task properties i give the form key of the form i made and execute the process the Activiti App displays a message stating " Sorry an error has occurred." but no exception is shown on the tomcat terminal.

Please advice where am i going wrong

Hi Asheer Abbas ,

Please have a look at the documentation for how to setup user task (Alfresco Activiti) and make sure you follow it correctly.

Use the 'Reference Form' configuration setting to associate a form to a user task instead of Form Key.
