As always, I can only recommend Jeff Potts' tutorial on custom advanced workflows, specifically the section about configuring Share.

I don't know what you want in terms of "how to create my own forms correctly" - what is "correct" and what is not in terms of contents of the form depends on the use case of the process, whereas the technical practice is documented and simply needs to be followed.
