The meta-data you describe is available on the version store nodes stored for each version of the document. It is possible to extract that data using the VersionService java API and in Alfresco 2.1 it will be possible to extract that information using a FreeMarker template - as the version history for a node is now available in the data-model. You can use the javascript API to change the content of a document (textual content) so in theory you could change the content to include that information.
