Image preview Dashlet not displaying images

Working on 4.0c test server and found that existing (and new) Image Preview Dashlet instances aren't displaying any images. I am able to view the thumbnail images in the document library but not on with the site dashboard dashlets. Not sure if anyone else has come across this issue. I am using 4.

As part of troubleshooting, I went ahead and did a clean install of 4.0a. I created a new empty MYSQL database and did not import old data stores. I created a test site, uploaded two jpg images and added the image preview dashlet to a dashboard. Here is the error displayed in the dashlet window
