support came back to me:

"CopyActionExecuter" class has changed in 4.0.2 and unfortunately the sample xml file doesn't reflect that. I'll raise a JIRA to fix the issue.

In the mean time please remove "assoc-type" and "assoc-name" from 'copyAction'.
