I have experienced an issue with Alfresco Share where uploading a new version of a document does not replace the old version, although the history information for both are kept. Im working with Alfresco share 3.1.1 enterprise on windows2003 / mysql.
Haven't come across this issue.

Maybe the viewer/browser caches the older version ?
Have you tried to log out and log back in to alfresco share.
Or close browser and open a new one. Log back in to share.
