Has anyone answered this? I also need to create permissions based on category. I'd like it to work so that the user has to have permissions to EVERY category that the file is labeled with in order to have access to it. Is that possible?

This functionality has actually been implemented in the Records Management module. It's called Supplementary Markings. If you are feel adventurous you can try and adapt that functionality.
